Important Information for filling in the form
Age | You must be 18 or above in order to pledge your organs. |
Witness | The witness must be age 21 and above. It is also recommended that one of your witnesses be a family member. |
Signature | Please ensure that all signature fields are signed otherwise we will not be able to process your request. |

Withdrawing your Objection to HOTA
❗Note❗
Upon acceptance of your withdrawal, you will continue to be given a lower priority on the waitlist for two years, compared to a person who had not registered an objection.

Objecting to HOTA
❗Note❗
Those who opt out of HOTA will be given a lower priority on the waiting lists if they need an organ transplant. Should you withdraw your objection later on in life, you will continue to be given a lower priority as a proposed recipient, compared to a person who has not registered an objection, for 2 years after the date your withdrawal is received.
