How can I make my decision known?
The decision to donate your organs is completely yours to make.

If you know the answers to the questions above, read on to see the various ways you can make your decision.

✅ I wish to donate
Remain in HOTA
No form submission required.
Opt in to MTERA
Complete the yellow Organ Donation Pledge form and submit it digitally via “NOTU Organ Donor Registry Services” or mail it to NOTU.
If you have previously opted-out of HOTA and wish to withdraw your objection:
Complete the blue Withdrawal of Objection to Organ Removal form and submit it digitally via “NOTU Organ Donor Registry Services” or mail it to NOTU.
Note: Individuals may withdraw their objection at any time. However, the individual will continue to be given a lower priority on the waitlist, compared to a person who has not registered an objection, for 2 years after the date the Director of Medical Services (MOH) receives the individual’s withdrawal.

Opt out of HOTA
Complete the pink Objection to Organ Removal form and submit it digitally via “NOTU Organ Donor Registry Services” or mail it to NOTU. Individuals can opt out for all or specific organ(s) listed under HOTA.
Note: Individuals who opt out of HOTA will get a lower priority on the organ transplant waiting list than those who remain in HOTA if they require an organ transplant in the future.
